Urgently Hiring – Registered Nurse (RN)
Full-Time - Multiple Territories Available

Health Link Home Health and Hospice has been a trusted provider of comprehensive home health services throughout the greater San Francisco Bay Area and East Bay for nearly two decades. We are expanding our team and are seeking compassionate, organized, and team-oriented Registered Nurse (RNs) for multiple territories.

Available Territories & Pay Structure
We are hiring for three separate territories. Compensation is based on experience and territory assignment:
- Alameda County & West Contra Costa County
Pay Rate: $125,000 – $146,000 per year DOE - Eligibility of a $5,000 Sign on Bonus for Full-Time status employees
Coverage Area: Oakland/San Leandro/Berkeley/Richmond/San Pablo - East Contra Costa County
Pay Rate: $125,000 – $146,000 per year DOE
Coverage Area: Antioch/Pittsburg/Brentwood

Responsibilities
- Provide skilled nursing services within the RN scope of practice and in accordance with the individualized plan of care
- Follow the established plan of care, deliver treatments, evaluate patient response, and promptly report any changes in condition to the Physician, Case Manager, and/or Director of Nursing
- Maintain accurate, timely clinical records and documentation in accordance with organizational policy using the company-issued device
- Provide supervision to Licensed Vocational Nurses (LVNs) when assigned, ensuring adherence to the plan of care and quality standards
- Collaborate with the Interdisciplinary Team to support coordinated, patient-centered care
- Follow all company policies and procedures
- Perform other duties as assigned

Requirements
- Current California RN License (Required)
- Home Health Experience: 1 year (Preferred)
- Current CA Driver’s License
- Excellent organizational and communication skills
- Ability to work independently in the field
