Description of Work
The Human Services Provider Program Coordinator II serves as the Home Manager and oversees all program coordination and staff supervision within a certified Intermediate Care Facility/Intellectual Disabilities (ICF-ID). In this role, they act as the Team Leader for assigned residents, ensuring each individual receives comprehensive training, services, and supports that promote maximum independence and protect their rights. They design and write objective program plans tailored to residents’ needs and monitor the implementation of these programs. The coordinator also provides hands-on programming and trains staff to ensure consistent, high-quality service delivery. Additionally, they are responsible for supervising all Residential Services staff assigned to the home, ensuring effective operations and resident care.This Home Manager position is located within the Rolling Hills Division with it's working hours being Monday - Friday from 8:00am-5:00pm as needed based on the needs of the facility.

Salary Range: $41,206.00 – $72,113.00

Thorough knowledge of the concepts, practices, and techniques of the field of developmental disabilities.
Possess skills in individual treatment planning such as person-centered planning, individualized education planning and/or individual service planning and outcome documentation.
Demonstrated ability to coordinate professionals, paraprofessional, and direct support staff in the development, implementation and evaluation of person-centered services.
Demonstrated ability to manage and supervise the operation of a residential unit and to develop and maintain cooperative working relationships with the residents, families and staff.
Physical Requirements:
Must be able to lift a minimum of 50 pounds on a regular basis (6-8 times daily). Must be able to stoop, bend, walk, run short distances (50 yards) and climb stairs without limitations of a permanent nature.
This Position Is Designated As Mandatory And Must Report Or Stay Over During Adverse Weather Events.

Veteran’s and National Guard Preference

Bachelor’s degree in a human services or related field from an appropriately accredited institution and two years of experience providing professional or direct services (either paid or volunteer) in a setting that serves individuals with disabilities.
