Nurse Manager – PACE Program

Trinity Health PACE is seeking a dedicated and mission-driven Nurse Manager to join our interdisciplinary team supporting older adults in our Program of All-Inclusive Care for the Elderly (PACE). POSITION PURPOSE

The primary purpose of your job position is to coordinate and supervise nursing services provided to the participants in the PACE Organization (PO). Services may be delivered in the day center or in participants’ homes.

ESSENTIAL FUNCTIONS

Key responsibilities include demonstrating the Mission, Vision, Values, and Guiding Behaviors of Trinity Health and TH PACE; fostering a collaborative, warm, participant‑focused environment; serving as a leader within interdisciplinary care; maintaining department budgets; hiring, coaching, and developing staff; ensuring regulatory compliance; managing documentation and infection control; coordinating participant assessments; supporting QAPI initiatives; collaborating with contracted providers; maintaining survey readiness; and participating in administrative on‑call rotation.
 

MINIMUM QUALIFICATIONS

• Current, unencumbered RN license in state of PACE Organization; BSN required
• Active CPR certification
• Minimum 3 years’ health care experience, with emphasis in geriatrics
• Minimum 1 year experience with frail/elderly population (or commitment to training)
• Previous supervisory experience required
• Valid driver’s license and proof of insurance
