  • Collaborative Culture: Join a team where loyalty, mentorship, and shared success drive everything we do.

What You’ll Do

  • Deliver personalized, high-quality care to older adults in homes and senior living settings.
  • Conduct comprehensive assessments and develop individualized care plans.
  • Manage chronic conditions with skill and empathy.
  • Collaborate across clinical disciplines (primary, psych, palliative, neuro) for whole-person care.
  • Build trusted relationships with residents, caregivers, and community partners.
  • Maintain thorough documentation and consistent communication.

What You’ll Bring

  • Active, unencumbered NP or PA license in the state you will be practicing.
  • Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
  • Compassionate, patient-centered approach and strong clinical judgment.
  • Valid driver’s license and reliable transportation.

  • Comprehensive Benefits: Health, dental, vision, 401(k).
  • Flexible Schedule: Balance purpose, autonomy, and stability.
