EMPLOYMENT RELATIONSHIP: Just cause subject to a probationary period
CURRENT HOURS AND STARTING TIME: Full-time (37.5 hours per week) position. The starting time for this position is 8:30 a.m.
GENERAL SUMMARY
Assists clients for Medicaid enrollment and other government funded programs. Schedules appointments and provides care coordination. Provides interpretation services for Spanish speaking clients at the clinic, over the phone and the home. Translates written materials into Spanish. Advocates for clients and make referrals as needed.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provides direct outreach services at community sites by promoting agency services, encouraging participation in programs, promoting healthy lifestyle behaviors, and making referrals to resources.
Schedules appointments and provides coordination of resources in MI Bridges.
Identifies potential high risk clients in clinics and the community.
Provides guidance and assistance to clients in obtaining health and social services, including Medicaid application assistance.
Provides basic education on health promotion and prevention behaviors.
Advocates for clients and makes referrals as needed.
Reinforces teachings of Public Health Nurses and Social Workers.
Provides services in a culturally appropriate manner.
Provides independent home visits as directed.
Maintains accurate records and completes forms relevant to the program.
Assists in collection of data and compiling records and reports.
Provides follow up on abnormal health screenings.
Performs related work as required.
QUALIFICATIONS
Required Education and Experience
- High school diploma (or GED)
- One (1) year of experience in Community Health and Human Services
Required Licenses or Certifications
- Possession of a valid Michigan driver’s license and operable, insured automobile for authorized travel
