In this valuable role, the Clinical Unregulated Care Provider Supervisor is accountable for the supervision of PSWs and HSWs who are caring and making a difference for clients within the community. The Clinical UCP Supervisor will have the expertise to assess, implement and evaluate a wide range of clinical care interventions to ensure the delivery of safe, quality, and ethical care.

What you’ll do:  

  • Supervise and support PSWs and HSWs to ensure practices comply with all legislated standards of practice as well as company policies and procedures.  

  • Respond to and advise/resolve concerns from community staff, patients/families, and other team members.  

  • Provide ongoing leadership through regular huddles, face to face meetings and one-on-ones.  

  • Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.  

  • Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.  

  • Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals. 

  • Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.  

  • Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.  

   

What you bring:  

  • Current RPN or RN registration with the College of Nurses of Ontario and entitled to practice without restrictions  

  • Minimum of 5 years experience as a practicing Registered Nurse or Registered Practical Nurse  

  • Comprehensive patient health assessment skills are required  

  • Possess an understanding of the principles of community nursing practice  

  • Proven experience leading and managing a team 

  • Knowledge of clinical documentation standards and practice guidelines is required  

  • Excellent organizational, interpersonal, and teaching skills  

  • Commitment to professional development in clinical practice that is relevant to home health care  

  • Effective critical thinking and problem-solving skills  

  • Must possess a valid driver’s license and access to reliable transportation  

  • Able to provide a current, clear Vulnerable Sector Screening (VSS)  

  • C.N.A. Certification as a Community Health Nurse (CCHN(C)) is considered an asset
