Position Overview
The Clinical Care Manager – Licensed Nurse (LPN/LVN) supports the coordination and delivery of patient care in a home health setting. This role partners closely with clinicians, physicians, and internal teams to ensure timely, high-quality care and an exceptional patient experience.
What You’ll Do
Care Coordination & Patient Support
- Coordinate care across interdisciplinary teams to support patient outcomes
- Manage physician orders and ensure timely processing and approvals
- Support referral intake and follow up to ensure timely admissions
- Participate in case conferences and contribute to care planning decisions
- Review patient reports (occurrences, infection control) and escalate concerns appropriately
- Conduct in-home visits as assigned and document visits accurately
Communication & Documentation
- Maintain timely, accurate clinical documentation in the EMR
- Communicate effectively with physicians, providers, and internal team members
- Collaborate with field staff to ensure alignment on patient care plans
Quality & Operations
- Identify workflow or system gaps and partner with leadership on solutions
- Support compliance with infection control standards and best practices
- Participate in training, education, and ongoing professional development
What You’ll Bring
- Active LPN or LVN license in the state of practice
- 2+ years of healthcare experience (home health preferred)
- Strong clinical judgment and attention to detail
- Excellent communication and coordination skills
- Ability to manage multiple priorities in a fast-paced environment
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
