Job Title:  Physical Therapist
Department: Physical Therapy - Inpatient & Outpatient
Location: E.J. Noble Building - 80 E. Main St., Canton, NY 13617
Schedule: Day shift, 8-hr. shift between 7:00 am - 5:00 pm; Monday - Friday and weekend availability preferred  
 

SUMMARY:

The physical therapist completes patient assessments, establishes patient care plans, administers patient treatment, and directs delivery of treatment utilizing professional physical therapy assistants in accordance with licensure and regulatory scope and standards, and established hospital policies, procedures, and protocols. Adheres to the professional standards of CPH, the American Physical Therapy Association, and NYS Office of Professions. Actively supports and participates in patient coverage efforts across all departments as necessary. Hours may vary, including weekend rotation for the inpatient department.

RESPONSIBILITIES: 

  • Conducts comprehensive physical therapy evaluations and develops individualized treatment plans to restore function, manage pain, and prevent disability.

  • Administers and adjusts therapy interventions, provides patient and caregiver education, oversees assistants, and ensures smooth discharge planning and continuity of care.

  • Collaborates with healthcare providers, participates in quality improvement and ongoing professional development, and completes all required documentation.

REQUIRED QUALIFICATIONS:

  • Bachelor's, Master's, or Doctorate required. Effective 07/24/2025. If a Bachelor's degree is held, 1 year of prior experience is required.

  • Current New York State license or a limited permit as a Registered Physical Therapist

  • If a New York State license or limited permit is not held, must be hired within 90 days of graduation from New York State or APTA-accredited physical therapist education program and meet Practice Exemption under New York State Physical Therapy License requirements.

  • If working in home care or traveling between locations, NYS license required

 

PREFERRED QUALIFICATIONS:

  • Doctorate in PT, or PhD in P.T.

  • Minimum of 1 year of post-licensure practice as a Physical Therapist

Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20–50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask-fit testing, and physical exams are common requirements.
