St. Peter’s Health Partners Medical Associates has a unique and rewarding opportunity for an LPN to join our team.

This LPN opportunity is with our Home Based Primary Care Team: A team of NPs/PAs delivering primary care in patient homes to those who cannot get to a provider’s office.  In the office, located at 433 River Street in Troy, NY, is our Office Manager and RN.  Our Home Based Primary Care team serves a vulnerable patient population, making a significant difference in their and their caregiver’s health outcomes and quality of life.  Our service area covers parts of Albany, Rensselaer, Saratoga and Schenectady counties.

What you will do: 

The LPN working with our Home Based Primary Care Team will work alongside our practice RN, spending most of the time in our Troy office, with occasional visits to our home bound patients.  In the office, the LPN will support the team and work closely with our Office Manager and RN to meet patient/family needs. Office tasks include answering telephones, triage patient calls to providers and nurse, scheduling, referral coordination, and reviewing lab results with patients. Occasional home visits may include phlebotomy and other specimen collection, and administration of medications and vaccines.   

 

 

What you will need: 

·        A current license to practice as a Practical Nurse in the State of New York 

·        Graduate of accredited school of Practical Nursing. 

·        High School diploma / GED required 

·        Current New York State Drivers License and the ability to drive to patient homes to deliver care

        Must be able to lift 20 lbs. 

 

Pay Range: $23.85 - $29.95 

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
