Remote RN Case Manager (Illinois License Required)

Are you a compassionate RN looking to make a meaningful impact—without leaving your home? Join our team as a Remote Case Manager RN and help improve the health and wellness of members through proactive, holistic care coordination.

What You’ll Do

• Manage member care through telephonic assessments and case management
• Develop personalized care plans to improve health outcomes
• Identify risks, remove care barriers, and support complex health needs
• Coordinate with multidisciplinary teams to ensure seamless care
• Use clinical expertise and data insights to guide decision-making

Schedule

• Monday–Friday, 9:00 AM–5:30 PM (based on your time zone)
• Occasional evenings, weekends, or holidays as needed

What You’ll Bring - REQUIRED

• Active Illinois RN license (required; no IL residency needed)
• 2+ years of RN clinical experience (all specialties welcome)
• Strong communication, assessment, and care coordination skills
• Associate’s Degree in Nursing

Nice to Have

• Case management or managed care experience
• CCM certification
• BSN
• Advanced proficiency with Microsoft Office

Pay Range

The typical pay range for this role is:

$66,575.00 - $142,576.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
