Advanced Clinical Provider – Hospital at Home Program

Hospital at Home is a unique and innovative program at NYP that provides inpatient hospital level care at the patient’s home. The program combines daily in-home visits, virtual visits, and 24/7 remote patient monitoring to give eligible patients an opportunity to stay in the comfort of their own home while receiving their hospital care. This program will be designed and delivered by NYP providers and clinical staff, allowing NYP's best-in-class care across all measures of quality, service, cost, and innovation to be continued in the home setting. Hospital at Home will provide strategic value to NYP by alleviating capacity constraints, improving patient experience, reducing readmission rates, and driving cost savings to the organization. Since 2020, CMS has authorized more than 320 hospitals across 133 health systems in 37 states to offer acute hospital care at home, and those numbers are climbing. The number of programs is expected to double by 2026. We are looking for motivated team members to help us become the largest, safest, and most innovative Hospital at Home program in the country.
We are currently seeking Advanced Clinical Providers (ACPs) to join our Hospital at Home (HAH) team. In this vital role within our Care Team Model, ACPs deliver high-quality, patient-centered care to patients receiving acute-level services in the home setting.
Key Responsibilities:
Provide clinical care to HAH patients under the direction of the Hospital at Home Medical Directors, Supervising Physician, and Associate Chief Advanced Clinical Provider
Deliver high-quality clinical care to HAH patients across emergency departments, inpatient units, and the Virtual Unit.
Manage the full continuum of care for HAH patients, including enrollment, admission, daily clinical evaluations, discharge, consultative services, clinical monitoring, and telehealth visits
Demonstrate an ability to make appropriate, highly complex clinical decisions and take effective action
Perform on-going assessment of therapy and modify patient management plan as necessary
Collaborate with interdisciplinary team members to ensure continuity of care and optimize the use of hospital resources.
Assists the physician as directed in the training of health personnel in diagnostic, therapeutic and clinical techniques.
Participate in continuing education and professional development activities to maintain clinical excellence.
**Shift: This is 24/7 service; three 12.5-hour shifts; MAINLY DAYS WITH MINIMAL NIGHTS (1-2 shifts a month) including weekends, and rotating holidays.
Required Criteria
Graduate of an accredited PA program
Current NYS PA licensure
NCCPA certification
DEA Certification
BLS/ACLS/PALS certification
At least one year of relevant clinical experience as a physician assistant
Preferred Criteria
Clinical experience in Inpatient Hospitalist Medicine and/or Emergency Medicine
