Highlights:
Summary: The Palliative Care Coordinator provides daily direct patient care and guidance for the palliative care team, acts as a liaison between palliative care and the hospital staff and management to improve patient care, education and marketing and to ensure future development of the palliative care program within the Baptist Health system and the community. Education / Experience:Degree in Nursing, Bachelors degree preferred. Minimum 2 years hospice and/or palliative care experience required. License / Certification: Active RN license required, BLS required, Hospice & Palliative Care certification preferred Knowledge, Skills, and Abilities:• Strong team leadership skills, strong organizational, interpersonal and customer relations skills
• Compliance with regulatory agencies and promotion of Baptist Health mission, vision and values.
• Strong written and oral communication skills, proficient computer skills,
• Proven track record of obtaining, managing, and using information to improve patient and family outcomes.Working Conditions/Physical Requirements: Position requires sitting for 10% of the workday and mobility 90% of the workday.
There is occasional stooping, bending, kneeling, and crouching.
There is frequent reaching, use of hands and repetitive action.
Requires occasional lifting or carrying up to 25 pounds, pushing/pulling up to 150 pounds.
There is a continuous need for speaking with clear diction and hearing sensitivity in conversation either by phone or in person.
Continuous visual acuity at close and distant range.
Inside protected from weather conditions but not necessarily from temperature changes. Minimal noise levels.
Much exposure to computer work.
Occasional exposure to risk of electrical shock.
Some exposure to infectious disease while performing job-related tasks.
Some exposure to hazardous chemicals. Occasional exposure to radiation.
