We are currently looking for a compassionate, organized, and team-oriented Full-Time Home Health LVN to serve patients throughout San Francisco. In this essential role, you will collaborate closely with our Director of Nursing, clinical staff, and office team to ensure coordinated, high-quality care.

Responsibilities

  • Implement the plan of care initiated by the Registered Nurse

  • Provide accurate and timely documentation consistent with the plan of care

  • Assess and educate patients and caregivers regarding diagnosis, care plans, and treatments

  • Participate in coordination of home health services, reporting additional needs (HHA, PT, OT, ST, MSW) to the RN or Clinical Supervisor

  • Use equipment and supplies effectively and responsibly

  • Complete online charting in Kinnser using company-provided devices

  • Follow all company policies and procedures

  • Perform other duties as assigned

Requirements

  • Minimum 1 year of nursing experience in a medical setting

  • IV certification and experience with wound care

  • Home health experience recommended but not required

  • Active California LVN license

  • Valid California Driver’s License

Job Type: Full-Time

Pay: $93,000 - $115,000/year DOE

This is NOT a contract position
