Job Summary
Infinite Medical Group is seeking a dedicated Palliative Care Physician to provide comprehensive, person-centered palliative and symptom-management services to residents in long-term care settings. This role is ideal for a physician passionate about improving quality of life, guiding goals-of-care discussions, and supporting residents and families as they navigate complex medical decisions.
Responsibilities
- Deliver palliative and supportive care services to residents in skilled nursing and long-term care facilities
- Conduct comprehensive assessments focusing on symptom management, functional status, and quality of life
- Manage complex chronic and advanced illnesses with a comfort-focused, patient-centered approach
- Lead goals-of-care and advance care planning conversations with residents and families
- Collaborate with interdisciplinary teams, including facility staff and primary care providers
- Develop, implement, and monitor individualized symptom-management and treatment plans
- Order and interpret diagnostic tests when clinically appropriate
- Prescribe medications and evaluate ongoing treatment effectiveness
- Educate residents, families, and facility staff on disease progression, treatment options, and care expectations
- Complete all required clinical documentation accurately and in a timely manner
- Maintain a compassionate, respectful, and supportive care environment
Requirements
- Active and unrestricted MD license
- Minimum 2 years’ experience in palliative care in a hospital or nursing home–based program
- Certification in palliative care is a plus
- Board Certification or Board Eligibility in Palliative Medicine, Hospice & Palliative Medicine, Internal Medicine, Family Medicine, Geriatrics, or a related specialty
- Strong communication skills with the ability to navigate sensitive and emotionally complex situations
- Commitment to elder care, comfort-focused medicine, and interdisciplinary collaboration
