Concord Post Acute is seeking a motivated and experienced Director of Rehabilitation (DOR) to lead our rehabilitation department and help ensure our residents receive high-quality, patient-centered therapy services.

Position Overview
The Director of Rehabilitation is responsible for the day-to-day leadership and operations of the rehabilitation department. The DOR provides clinical guidance, supervises rehabilitation staff, coordinates scheduling and caseloads, ensures regulatory and documentation compliance, and works closely with facility leadership to support excellent resident care.
The DOR will oversee PT, OT, SLP, therapy assistants, aides, and students while maintaining a collaborative relationship with nursing, administration, physicians, residents, families, and referral partners.
Key Responsibilities
Lead, supervise, and support all rehabilitation department staff
Coordinate daily staffing and scheduling to meet resident and facility needs
Provide direct patient care while managing administrative responsibilities
Ensure residents' rehabilitation needs are appropriately assessed and addressed
Monitor the quality and effectiveness of therapy services and treatment programs
Develop and implement new clinical programs based on resident and facility needs
Ensure compliance with regulatory requirements and company policies
Review clinical documentation for accuracy, completeness, and timeliness
Monitor department productivity and established performance standards
Support denials management and ensure billing and documentation requirements are met
Approve department timecards and review staff time worked
Participate in staff performance evaluations and provide feedback to leadership
Coordinate orientation and onboarding for new rehabilitation team members
Provide ongoing education and in-services related to safety, body mechanics, clinical practices, and workplace safety
Foster positive relationships with facility staff, residents, families, and referral sources
Participate in facility and company meetings
Identify opportunities to improve rehabilitation services and department operations
Assist with developing new referral and business relationships
Promote a positive, professional, and team-oriented rehabilitation environment
Maintain confidentiality of resident and company information
Pursue ongoing professional development and continuing education
Qualifications
Licensed PT, OT, or SLP eligible to practice in California
Bachelor’s degree in Physical Rehabilitation, Occupational Rehabilitation, or Speech-Language Pathology preferred
Master’s or Doctorate degree preferred
Previous rehabilitation leadership or supervisory experience strongly preferred
Strong clinical judgment and organizational skills
Excellent communication and interpersonal skills
Ability to effectively lead, mentor, and motivate a multidisciplinary rehabilitation team
Strong understanding of documentation, billing, compliance, and quality standards
Proficiency with electronic documentation systems, including Net Health and PointClickCare, preferred
Ability to work collaboratively with facility leadership and interdisciplinary teams
Commitment to providing high-quality, patient-centered rehabilitation services
