Summary
The Patient Flow Coordinator I is responsible for ensuring patients are placed in a timely manner across the TJUH, Inc. Division. Coordinates and communicates with personnel and departments, including Medical Staff, Nursing, Environmental Services, Patient Transportation and the Jefferson Transfer Center. Participates in bed coordination for the most efficient bed utilization 24 hours per day/ 7 days per week.

Job Duties

• Maintain patient throughpout and continually evaluate bed placement progress according to the institution’s Patient Flow Plan. Coordinate Direct Admissions, including urgent and scheduled, and contacts patients and family members when the appropriate bed is available. Identify and communicate quality and risk issues to the appropriate staff/ departments. Monitor the EVS and Patient Transport Command Center to ensure that bed cleans and patient transports are assigned according to bed prioritization needs. Specific Actions/Decisions:
• Assign beds to patients from all clinical departments, direct admissions, and external transfers based on defined workflow. Escalate potential administrative issues resulting in barriers to discharge and delays in care to appropriate PFMC and C.O.R.E. leadership.
• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.

Minimum Qualifications

• High School Diploma/GED
• 1 year Healthcare/ Medical – Admissions
• Performs duties in an independent manner based on guidelines set by supervision.
• Customer Service, Critical thinking skills
• Problem solving, relationship building, interpersonal

Preferred Qualifications

• EMR Skills and Knowledge

Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.

Salary Range

$17.00 to $23.64 Hourly

The actual hiring rate will be determined based on candidate experience, skills and qualifications. This position is not eligible for an annual incentive.
