About the role
Essential Functions (Duties and Responsibilities):
Adherence counseling, harm reduction, health promotion, and patient navigation
Collect all required data elements and provide timely reports
Outreach services to persons living with HIV, through direct personal contact and collaborative community partnerships
Linkages with Meharry Community Wellness Center, community-based organizations and specialty services
Attend all mandatory meetings
Perform other related duties as assigned by Supervisor or Executive Director.
Knowledge, Skills and Abilities:
Basic knowledge of HIV/AIDS
Be a team player and well-organized
Skills in engaging people living with HIV
Ability and willingness to use both knowledge and personal experience in facilitating linkage of individuals and groups to essential HIV health related services
Communication skills and proven behaviors consistent with maintenance of health and wellness.
Ability to establish trust based relationships with people in an appropriate and ethical manner
Knowledge and ability to access community resources
Ability to attend mandatory staff meetings
Basic computer skills and ability to collect data elements and generate reports in a timely manner
Motivated to serve people with HIV and get trained as assigned by Executive Director or supervisor
Education and Experience:
High school diploma with commensurate experience; associate or bachelor degree welcome
Familiarity with people living with HIV is mandatory
Familiarity with Nashville/Davidson and/or surrounding Counties
Familiarity with Ryan White programs and Meharry Community Wellness Center is highly desirable
Must possess valid driver’s license
Willingness to attend all required trainings
Reliability, punctuality, and discipline

Usual office environment.

Able to sit, stand or walk for extended periods of time
Able to travel in Nashville/Davidson and surrounding Counties
