About the role:
Our Hospice Admissions Registered Nurse serves as the first clinical point of contact for patients and families entering hospice care. This role is critical in guiding families through the decision-making process, delivering compassionate education, and ensuring timely and smooth transitions into care. With empathy, clinical expertise, and strong communication, the Admissions Nurse helps patients feel informed, supported, and comforted from the very beginning.
Job Responsibilities:
- Respond to hospice referrals with urgency, compassion, and professionalism
- Complete comprehensive nursing assessments to determine hospice eligibility based on regulatory and clinical criteria
- Provide education and support to patients and families about the hospice philosophy, services, and care expectations
- Obtain informed consent and initiate the hospice plan of care in collaboration with the interdisciplinary team
- Coordinate seamless admissions, including transition of care from hospital, facility, or home settings
- Communicate effectively with referral sources, physicians, and care partners to ensure understanding and alignment
- Document all assessments, communications, and decisions in the electronic health record (EHR) in real time
- Serve as a trusted clinical guide during a highly sensitive and emotional time for families
- Maintain flexibility to support same-day and after-hours admissions, as needed
Job Qualifications:
1. Current and unrestricted RN license in the state of Ohio
2. Minimum 1–2 years of nursing experience, with strong assessment and critical thinking skills
3. Valid driver’s license, auto insurance, and reliable transportation
4. Ability to work independently in the field
5. Strong interpersonal communication skills
6. Empathy and emotional intelligence in difficult conversations
