Job Title: Physical Therapist - Per Diem
Department: CPH Physical Therapy
Location: 49 Lawrence Ave. Potsdam, NY 13676* Must be ok with traveling to other locations
Hours Per Week: Per Diem
Schedule: 4 to 7.5 shift duration; shift varies based on availability
SUMMARY:
As a Physical Therapist, you welcome patients and put them at ease. You provide PT to patients with care and compassion to ensure rehabilitation success.
RESPONSIBILITIES:
Maintains established policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards.
Evaluates patient upon initial encounter to assess impairment and current condition to estimate rehabilitation potential.
Plans physical therapy treatment program for each patient including training in functional mobility, use of assistive devices, therapeutic exercise, balance and coordination retraining, massage, heat, and cryotherapy.
Applies diagnostic and progressive muscle, nerve, joint and functional ability tests.
Directs and aids patient in therapeutic regimen, delegating routine care to supportive personnel as appropriate. Acts in supervisory role to ensure proper execution of tasks.
Adapts conventional physical therapy techniques to meet the needs of patients as necessary.
Conducts and participates in training medical and nursing personnel in physical therapy techniques and objectives.
Evaluates, records, and reports on patient progress for review by other members of the rehabilitation team. Communicates effectively with physician, other members of the rehabilitative team, and others indicated regarding treatment plan and patient’s response.
Counsels patient and family according to degree of disability or impairment.
Performs other duties as assigned.
Trains, mentors, and coaches team members to ensure standards of the department are being met.
Addresses scheduling needs to ensure proper coverage is provided.
Identifies and leads process improvement initiatives to improve compliance and create efficient and effective workflows.
Works collaboratively with other departments throughout the System to ensure the department’s needs are being met and goals are aligned.
Steps in for manager during absences.
REQUIRED QUALIFICATIONS:
Doctorate or Master’s Degree in Physical Therapy, or Bachelor’s Degree with prior experience as a licensed Physical Therapist required.
Current New York State license or a limited permit as a Registered Physical Therapist.
If New York State license or limited permit is not held, must be hired within 90 days of graduation from New York State or APTA accredited physical therapist education program and meet Practice Exemption under New York State Physical Therapy License Requirements.
If working in Homecare or traveling between locations, NYS license required.
PREFERRED QUALIFICATIONS:
Prior Physical Therapist experience preferred.

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
