Physical Therapist (PT) – PRN Weekends
Location: Sunnyvale Post Acute – Sunnyvale, CA
Pay Rate: $57–$67 per hour (DOE)
Schedule: PRN / Weekends

We are seeking a talented PRN Weekend Physical Therapist to join our rehab department.
Position Summary
The PRN Physical Therapist evaluates, plans, and provides patient‑centered physical therapy services for individuals requiring rehabilitation in a post‑acute/long‑term care setting. You will collaborate closely with an interdisciplinary team to ensure the best rehabilitation outcomes for our residents.
Responsibilities
Complete PT evaluations and develop individualized treatment plans
Provide effective, evidence‑based therapeutic interventions
Document treatments, progress notes, and evaluations in accordance with facility and regulatory requirements
Communicate patient progress with interdisciplinary team members
Educate patients, families, and caregivers on therapy goals, safety, and home programs
Ensure safe, ethical, and high‑quality care at all times
Qualifications
Current California Physical Therapist (PT) license
Graduate of an accredited PT program (DPT or MSPT)
Previous SNF or post‑acute rehab experience preferred (not required)
Strong clinical reasoning and communication skills
Ability to work independently and collaboratively
