Why Join Centria Healthcare?
At Centria Healthcare, we don’t just staff shifts—we transform lives. Founded in 2009, we have grown to become the #1 provider of Private Duty Nursing (PDN) in the state of Michigan. We specialize in providing exceptional, life-changing care to two vital populations right where they are most comfortable: in their own homes.
-
Our Pediatric Warriors: Children born with chronic respiratory conditions who rely on trachs, mechanical ventilators, and feeding tubes.
-
Our Adult Clients: Individuals recovering from catastrophic Motor Vehicle Accidents (MVA), Spinal Cord Injuries (SCI), and Traumatic Brain Injuries (TBI).
We understand that our nurses are our greatest asset. That’s why we explicitly structure our organization to support a healthy work-life balance, ensuring our clients and staff stay healthy, happy, and supported. If you love what you do and want to lead a dedicated team of field clinicians, we want to invest in you.
The Role: How You’ll Make an Impact
As a Clinical Nurse Manager, you are the bridge between clinical excellence and compassionate care. You will step into a pivotal leadership role, managing an assigned client caseload and supervising, coaching, and mentoring our incredible field staff (RNs, LPNs, and HHAs) to achieve outstanding patient outcomes.
This isn't just an administrative job—it’s a dynamic role where you get to use your clinical expertise to educate families, support field staff, and ensure the highest standards of safety and care.
Key Responsibilities:
-
Lead & Mentor: Provide clinical education, coaching, and mentorship to field staff, building strong relationships and offering real-time feedback.
-
Clinical Oversight: Perform case opens, conduct medical assessments, manage medication reconciliations, and update individualized Plans of Care (POC).
-
In-Home & Telehealth Supervision: Proactively schedule and conduct up to 15 in-home supervisory visits per week, alongside participating in a rotating telehealth on-call schedule.
-
Collaboration: Work hand-in-hand with the Director of Clinical Services and our cross-functional healthcare teams to maintain top-tier compliance and client satisfaction.
-
Advocacy & Documentation: Review shift notes to track patient progress, manage supply inventories when needed, and document clinical status changes.
What We’re Looking For (Qualifications):
The Must-Haves:
-
Education: Bachelor’s degree required.
-
Licensure: Active, unencumbered Registered Nurse (RN) license in the State of Michigan.
-
Experience: Minimum of 1+ years of nursing experience.
-
Tech Savvy: Comfortable using laptops/desktops, Microsoft Office Suite, and G Suite (Google Docs, Sheets, Meet) to manage digital documentation.
The Nice-to-Haves (Preferred Experience):
-
Background in a home care or private duty nursing model.
-
Clinical knowledge or hands-on experience working with Spinal Cord Injuries (SCI), Traumatic Brain Injuries (TBI), respiratory failure, or pediatric populations (trach/vent/G-tube dependent).
Core Skills:
-
Superior customer service and interpersonal communication skills.
-
Highly organized with the ability to confidently navigate shifting clinical priorities.
-
A collaborative mindset built for interdisciplinary teamwork.
Working Conditions & Physical Demands:
-
Schedule: Centria’s standard office hours are Monday through Friday, 8:30 AM – 5:00 PM, with flexibility required to meet company objectives or handle on-call duties.
