Location: Hybrid; ElderONE - Silver Hill PACE Center | 1000 Technology Pkwy, Newark, NY 14513

Hours Per Week: 24

Schedule: Monday-Friday, 8:00 AM - 4:30 AM

SUMMARY

RN- Home Care promotes and maintains health of individuals in the community through teaching, counseling and appropriate preventive and rehabilitative services.  Responsible for assessment of health needs and contributing to the plan for care for individuals and families.  Responsible for the initial implementation of nursing care plans, education of paraprofessional staff, and provision of nursing care as well as the periodic reassessment of individual and family nursing needs.  Does related work as required. Must have availability of a motor vehicle.

RESPONSIBILITIES

  • Patient Care. Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care;

  • Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems

  • Evaluation & Documentation. Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff

REQUIRED QUALIFICATIONS

  • Nursing Diploma required

  • License and current registration to practice as Registered Professional Nurse in New York State

  • Current New York State Driver’s license and availability of motor vehicle

  • Excellent ability to follow direction and be flexible to change

  • Exceptional communication and interpersonal skills

  • Compassionate, warm and patient focused

PREFERRED QUALIFICATIONS

  • Associate of Science in Nursing (ASN)

  • 1 year of experience working with a frail or elderly population

  • 6 months to 1 year experience conducting UAS-NY Community Assessment preferred

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

$75,734.00 - $101,014.00
