Physical Therapist Assistant (PTA) – Home Health

Summa Home Health

Fulltime: 8 am- 5pm (Mon-Friday)

Territory: Clinton, Doylestown, Orville and surrounding areas

We are seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join our Home Health team. In this role, you will work under the supervision of a licensed Physical Therapist to provide skilled therapy services that help patients improve mobility, regain independence, manage pain, and enhance their overall quality of life in the comfort of their homes.

Position Summary

The Physical Therapist Assistant (PTA) provides skilled physical therapy services under the direction of a licensed Physical Therapist and in accordance with physician orders and the established plan of care. The PTA assists patients in improving mobility, reducing pain, restoring function, and achieving their rehabilitation goals while ensuring a safe and positive care experience.

Key Responsibilities

Patient Care

  • Provide skilled physical therapy treatments according to the Physical Therapist's plan of care.

  • Assist patients in improving strength, mobility, balance, endurance, and functional independence.

  • Implement therapeutic exercises, manual therapy techniques, and other prescribed interventions.

  • Monitor patient responses to treatment and communicate progress or concerns to the supervising Physical Therapist.

  • Help patients restore function and maintain optimal physical performance.

Patient & Family Education

  • Educate patients and caregivers on therapy techniques, safety precautions, and home exercise programs.

  • Instruct patients and family members on the proper use of assistive devices and equipment, including walkers, wheelchairs, canes, crutches, braces, and prosthetic or orthotic devices.

  • Encourage patient independence and promote safe mobility practices.

Care Coordination

  • Collaborate with physicians, therapists, nurses, and interdisciplinary team members to coordinate patient care.

  • Participate in case conferences and care planning meetings.

  • Support positive patient outcomes through effective communication and teamwork.

Documentation & Compliance

  • Complete clinical documentation accurately and timely in accordance with agency policies, Medicare requirements, and federal regulations.

  • Document treatments, patient progress, assessments, and educational activities.

  • Maintain compliance with professional standards and company policies.

  • Participate in chart audits and quality assurance activities as requested.

Professional Responsibilities

  • Attend required meetings, in-service trainings, and continuing education programs.

  • Participate in orientation and training of new employees as assigned.

  • Maintain active PTA licensure and required certifications.

  • Perform additional duties as assigned.

Qualifications

Required

  • Current Physical Therapist Assistant (PTA) license in the state of employment.

  • Minimum one (1) year of PTA experience in an acute care setting.

  • Valid driver's license and current automobile insurance.

  • Ability to travel throughout assigned service areas.

  • Strong communication, organizational, and clinical skills.

  • Ability to work independently while maintaining effective collaboration with the healthcare team.

Preferred

  • Previous home health or home care experience.

Physical Requirements

This position requires regular travel to patient homes and the ability to perform the physical demands associated with providing hands-on patient care.

Requirements include:

  • Frequent driving between patient visits

  • Standing, walking, bending, reaching, and lifting throughout the workday

  • Ability to lift up to 50 pounds with or without assistance

  • Ability to effectively communicate, document, and utilize electronic medical record systems

  • Ability to safely perform patient transfers and mobility assistance when necessary
