Physical Therapist Assistant (PTA) – Home Health
Schedule: Monday–Friday, 8:15 AM – 5:00 PM
Territory: Glendale, Greenhill's, N College Hill Surrounding area
Position Summary
The Physical Therapist Assistant (PTA) provides skilled physical therapy services under the supervision of a licensed Physical Therapist in accordance with physician orders and the established plan of care. This role focuses on helping patients improve mobility, restore function, reduce pain, and maximize independence in the comfort of their homes.
The PTA is responsible for delivering high-quality patient care, educating patients and caregivers, maintaining accurate clinical documentation, and ensuring compliance with all applicable regulations and professional standards.

Qualifications
Required
- Current Physical Therapist Assistant (PTA) license in the state of employment.
- Minimum one (1) year of PTA experience in an acute care setting.
- Valid driver's license and current automobile insurance.
- Ability to travel throughout assigned service areas.
Preferred
- Previous home health experience.

Key Responsibilities
Patient Care
- Provide skilled physical therapy services in patients' homes according to the Physical Therapist's plan of care.
- Implement prescribed therapeutic exercises, treatments, and interventions.
- Help patients improve mobility, strength, balance, and overall functional independence.
- Monitor patient progress and communicate changes in condition to the supervising Physical Therapist.
- Assist patients in restoring function and maintaining optimal physical performance.
Patient & Family Education
- Educate patients and caregivers on treatment plans, therapy techniques, home exercise programs, and safety precautions.
- Instruct patients and family members on the proper use of assistive devices, including walkers, wheelchairs, canes, crutches, braces, and prosthetic or orthotic equipment.
- Promote patient independence and self-management strategies.
Care Coordination
- Collaborate with physicians, therapists, nurses, and other healthcare professionals to coordinate care.
- Participate in interdisciplinary case conferences and care planning meetings.
- Assist with staff education and training related to physical therapy interventions when appropriate.
Documentation & Compliance
- Complete clinical documentation accurately and within required timeframes.
- Document treatments, patient assessments, progress, and educational activities.
- Maintain compliance with agency policies, Medicare guidelines, and federal regulations.
- Participate in chart audits and quality assurance activities as requested.
Professional Responsibilities
- Attend mandatory meetings, trainings, and continuing education programs.
- Participate in orientation and training of new team members as assigned.
- Maintain professional conduct and appearance while representing the organization.
- Perform other duties as assigned.
Essential Functions
- Travel to patient homes and assigned visit locations.
- Maintain regular attendance and punctuality.
- Complete and submit required documentation within company-established deadlines.
- Document routine visits, discharge visits, evaluations, recertifications, and resumption-of-care visits within 24 hours.
- Complete start-of-care documentation within 48 hours.
- Follow the established plan of care within the scope of practice for a licensed Physical Therapist Assistant.
