We are looking for a Statewide Contractor to coordinate and oversee the implementation of Initiative 5 within the Rural Health Transformation Program (RHTP). This position will be Full-Time and Onsite 3 Days/Remote 2 Days.
What You’ll Do
Lead the statewide design and refinement of the RHTP cardiometabolic health model, including prevention, screening, referral pathways, and community-based supports
Develop standardized tools, templates, workflows, and guidance documents for regional implementation
Establish program goals, KPIs, and performance measures aligned with CMS requirements and IDOH priorities
Ensure Initiative 5 integrates with other RHTP initiatives, including workforce, care coordination, and data/evaluation
Regional Implementation Support
Provide direct technical assistance to Regional Grant Managers and regional partners implementing cardiometabolic health strategies
Support regions in developing work plans, selecting evidence-based interventions, and building clinical-community linkages
Troubleshoot implementation challenges and provide real-time problem-solving support
Facilitate cross-region learning, consistency, and sharing of best practices
Clinical & Community Integration
Develop and promote models that connect clinical care (e.g., primary care, FQHCs, hospitals) with community-based prevention programs
Support regions in implementing lifestyle interventions, screening protocols, referral systems, and follow-up processes
Coordinate with statewide partners such as IRHA, IHA, IPHCA, and community organizations to strengthen cardiometabolic health infrastructure
Data, Evaluation & Reporting
Work with IDOH evaluation teams to define metrics, data collection processes, and reporting expectations
Review regional performance data and identify opportunities for improvement
Prepare summaries, briefs, and presentations for IDOH leadership and CMS as needed
Stakeholder Engagement
Build and maintain strong relationships with rural health partners, including hospitals, FQHCs, local health departments, EMS, community organizations, and academic partners
Represent Initiative 5 in statewide meetings, workgroups, and partner convenings
Communicate complex program requirements in clear, accessible language
Program Improvement & Innovation
Identify gaps, risks, and opportunities to strengthen cardiometabolic health outcomes statewide
Recommend policy, operational, or programmatic changes to IDOH leadership
Monitor national trends, evidence-based practices, and emerging models relevant to cardiometabolic health
What You'll Need
Required:
Bachelor’s degree in Public Health, Health Administration, Nursing, Nutrition, Exercise Science, or related field (Master’s preferred).
Experience in chronic disease prevention, cardiometabolic health, or community-clinical integration (5 Years)
Experience designing or implementing evidence-based health interventions
Strong understanding of rural health systems and community health models
Demonstrated ability to manage complex projects with multiple stakeholders
Excellent communication, facilitation, and relationship-building skills
Experience with CMS-funded programs or large-scale federal grants
Experience working with rural communities or underserved populations
Familiarity with lifestyle medicine, diabetes prevention, hypertension management, or related frameworks
Experience with program evaluation, data interpretation, or quality improvement
Physical Demands
Ability to safely and successfully perform the essential job functions consistent with the ADA and other federal, state, and local standards
Ability to conduct repetitive tasks on a computer, utilizing a mouse, keyboard, and monitor
Must be able to communicate with customers/team members over the phone and in person
Sedentary work that involves sitting or remaining stationary most of the time with occasional need to move around the office to attend meetings
In-state travel may be required

$44 - $46 per hourThe pay range for this position is listed above. Exact compensation may vary based on several factors, including location, experience, and education. Benefit packages include medical, dental, and vision insurance, as well as 401k retirement account access. Employees in this role may also be entitled to paid sick leave and/or other paid time off as provided by applicable law.
