Schedule Details/Additional Information:

This is a PRN position; however, the applicant is required to cover 1 Saturday per month at minimum.

Pay Range

$38.20 - $57.30

Major Responsibilities:

Patient Assessments / Reassessments
 Performs appropriate patient assessments resulting in an effective treatment plan.
 Reassesses patients at appropriate intervals of time demonstrating the ability to observe treatment effect and modify the plan of care as indicated.
 Involves the patient and /or family in the assessment process and the formation of the care plan.

Care Planning / Interdisciplinary Communication
 Provides care in accordance with physician’s orders.
 Demonstrates appropriate case management to ensure quality of care and best patient outcome.
 Communicates appropriately to physicians, nurses, case managers, and other members of the health care team to ensure care is collaborative and appropriate.

Patient Outcomes
 Assesses and documents patient outcomes criteria at the initial and discharge evaluations. (all shoulder, back, incontinence, and anodyne).

Speech Therapy Modalities / Patient Treatment
 Demonstrates knowledge of speech therapy modalities / treatments and performs correctly.

Operates Equipment Safely and Correctly
 Demonstrates knowledge of and competency in using equipment pertinent to providing speech therapy services.

Patient Education
 Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning.
 Includes the family in teaching as appropriate.

Documentation
 Documentation meets current standards and policies.
 Documentation is legible.
 No inappropriate abbreviations are used.
 Documentation is completed in a timely manner.
 Medical Records are closed as per policy.

Charge Capture
 Time is adequately documented in the assessments and daily treatment notes.
 Each charge bill adequately reflects all services provided.

Demonstrates self-growth as a professional therapist.
 Engages in continuing education and professional growth opportunities.
 

Licensure, Registration, and/or Certification Required:

Georgia Speech and Language Pathologist License.

Education Required:

Educational type and level as required by license.

Experience Required:

CFY will be considered.

Knowledge, Skills & Abilities Required:

Good interpersonal and communication skills, time management skills, and the ability to be flexible and dependable.

Physical Requirements and Working Conditions:

Must be able to use visual acuity to see monitor screen, computer and hard copy materials. Must be able to hear and verbally communicate in person and over the phone or radio. Must be able to sit for prolonged periods of time (up to 2 hours). Must be able to comprehend and learn operation of various office equipment. Must have functional range of motion of the cervical, thoracic and lumbar spines, upper and lower extremities with a grip strength of 50-60# specific to job evaluation. Must be able to forward reach, overhead reach, bend, squat, kneel and apply proper body mechanics during the transfers and transport supplies and/or equipment using proper body mechanics. Must be able to lift up to 10# specific to job evaluation..
