About the role

Physical Therapist Assistant (PTA) — Home Health Full-Time / Per Diem | San Francisco, CA Pay: $114,000 – $135,000 annually | $10,000 Sign-On Bonus

Where great PTAs come to do their best work.

At Health Link Home Health & Hospice, we believe great patient care starts with supporting the clinicians who deliver it. For over 20 years, we've served the Bay Area with a focus on clinical quality, strong systems, and a collaborative culture. We are expanding our San Francisco County team and are seeking a motivated PTA who wants meaningful work, modern tools, and a team that is truly present.

You'll serve patients across SF's diverse neighborhoods — from the Sunset and Richmond to the Mission and beyond — with a territory designed to minimize drive time and keep your day manageable.

Why PTAs Choose Health Link

  • AI-Assisted Charting: Smart documentation tools to slash your charting time and reclaim your evenings.
  • SF-Friendly Commute: Mileage reimbursement, a company vehicle, or rideshare credits (Uber/Lyft) — whatever works best for you.
  • Clinical Support: Experienced case managers and your supervising PT are available in real-time to help with scheduling, patient issues, or clinical questions.
  • Quality-Driven Bonuses: Our bonuses are tied to patient outcomes and quality of care, not just volume.
  • Stable Structure: Organized scheduling, manageable territories, and a team that respects your time and your profession.

Compensation & Benefits

  • Salary: $114,000 – $135,000 annually (based on experience)
  • Health: Medical, dental, and vision insurance
  • Retirement: 401(k) with employer matching
  • Perks: Paid time off, life insurance, pet insurance, company-provided mobile devices, and flexible scheduling

Your Role

  • Provide direct physical therapy services including gait training, therapeutic exercise, and neuromuscular re-education — under PT supervision
  • Implement individualized treatment plans to improve mobility, function, and safety in the home setting
  • Utilize physical agents such as ultrasound, heat/cold therapy, and massage
  • Instruct patients and caregivers on the use of wheelchairs, braces, crutches, and prosthetic devices
  • Complete documentation using AI-assisted tools
  • Collaborate with an interdisciplinary team to ensure exceptional patient outcomes

Requirements

  • Current California PTA license (required)
  • Valid California driver's license with a clean record (required)
  • Home health experience preferred — strong outpatient or inpatient PTAs are encouraged to apply
  • Strong communication skills, tech-savvy, and a compassionate approach to patient care
  • Ability to travel regularly within San Francisco County

Schedule

  • Monday through Friday
  • Weekend availability a plus
  • Full-time or per diem available
  • This is a direct hire position — not a contracted role
