Licensed Practical Nurses (LPN) among the Franciscan team are those who choose to focus on home healthcare are a special group of people that can provide direct patient care. In addition, this role holds the responsibility of educating caregivers and actively participating on the patient’s care team. This is a level of nursing that must be able to accurately assess needs, provide appropriate interventions, and revise care as needed under the supervision of a registered nurse. WHAT YOU CAN EXPECT
Minimum Start Pay $24.83 an hour
Implement nursing plan of care at the direction of the registered nurse.
Provide home-based patient care based on physician orders and the established nursing plan of care.
Collaborate with Home Health RN and multidisciplinary treatment team to individualize each patient’s treatment needs, to achieve defined goals for improved patient outcomes.
Conduct patient psychosocial assessments, to explore the physical, psychological, and social aspects of the patient and their conditions, obtain history, and elicit an accurate description of psychosocial needs.
Contribute to developing discharge plans, identifying the post-discharge needs of the patient, and target appropriate resources for follow-up care, in collaboration with the patient, family and care team.
Document patient care information, and ensure timely and accurate completion of patient care records in accordance with policies and procedures.
QUALIFICTIONS
Associate's Degree or Diploma from Accredited Licensed Practical Nurse Program - Required
3 Years Acute Care or Long-Term Care Experience - Required
1 year Home Health Experience - Preferred
Licensed Practical Nurse (LPN)- State Licensing Board - Required
Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required
Driver's License - Required
Auto Insurance - Required
