Job Summary: The Intake Coordinator comprehensively assesses cases at the time of eligibility to evaluate child/youth needs, and coordinates counseling for services as needed.


Essential Functions:
1. Comprehensively assess case needs immediately upon eligibility, including case clarification and review of services.
2. Develop assessment needs for the case and communicate these needs effectively with the FAYS team according to protocol and within the timeframe allotted.
3. In consultation with his/her supervisor, make decisions affecting the safety of the child.


Measurable Deliverables:
1. Meet all federal and state regulatory guidelines and standards that are applicable to this position.
2. Review, input and create FAYS billing reports within required timeframes, and in compliance with program quality expectations.
3. Contact clients and coordinate services through documented phone calls.
4. Conducts follow-ups with clients 75-105 calendar days after case closure.
5. Maintain accurate records, files, forms, statistics and additional relevant information in accordance with agency policy, licensing, and/or funding requirements.
6. Prepare, compile, and submit accurate client reports to the Program Supervisor.
7. Participate in workshops, seminars, education programs and other activities that promote professional growth and development.
8. Maintain confidentiality in all areas of the service population and program operations.
9. Maintain BCFS HHS professional and ethical standards of conduct outlined in the employee handbook including demonstrating respect for agency staff, the service population, and community members and complying with required dress code always.
10. Identify and maintain professional relationships with clients and community partners.
11. Attend and participate in staff meetings, in-service trainings and case consultations
12. Demonstrate the ability to:
a. Respond sensitively and competently to the service population’s cultural and socio-economic characteristics.
b. Communicate effectively in writing and verbally in English.
c. Work in a fast-paced environment.
d. Maintain computer literacy required to meet the responsibilities of the position.
e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
f. Travel as needed.
13. Other job duties as assigned per grant requirements.


Requirements:
1. Maintain certificates required for the position including but not limited to First Aid and CPR.
2. Valid Texas Driver’s license with a clear driving record and reliable transportation.
3. Pass a pre-employment drug screen and random drug screens.
4. Provide proof or work eligibility status upon request.
5. Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).
