Job Description

Schedule:

1 Day per Week- PRN

Monday - Friday, No Holidays or On Call

HOW YOU’LL WORK

In this role, you will:

  • Connect patients with community resources
  • Ensure patients’ medical, emotional, and safety needs are met
  • Deliver skilled services as ordered by the attending physician
  • Educate patients and family members
  • Promote patient safety and well-being

MAJOR AREAS OF RESPONSIBILITY

Plan of Care

  • Participate in developing patient care plans
  • Conduct patient assessments and identify barriers
  • Observe, document, and report patient changes to appropriate staff

Patient Outcomes

  • Connect patients and families to community resources
  • Assist with financial aid applications and long-term planning

Family Educator / Advocate

  • Counsel patients and families on coping, life changes, and grief

Policies

  • Complete clinical documentation in accordance with agency protocol and Medicare/Federal guidelines

Rules & Regulations

  • Follow all agency policies, procedures, and regulations
  • Communicate schedule or availability changes to supervisors

Operations

  • Attend required trainings and agency meetings

HARD & SOFT SKILLS

  • Compassionate communicator with a positive attitude
  • Strong patience when working with patients, families, physicians, and coworkers
  • Excellent attention to detail
  • Ability to follow directions and remain observant

REQUIREMENTS

  • Current Medical Social Worker license in the state of employment (limited licenses not accepted)
  • Minimum 1 year of MSW experience in an acute care setting (home care experience preferred)
  • Master’s degree in Social Work
  • Valid driver’s license and auto insurance
  • Ability to meet physical job demands
