GENERAL OVERVIEW:
Assesses patient need for diabetes education. Provides individualized diabetes self-management education/training based on assessment. Plans and conducts continuing education programs for professional staff and the community.
ESSENTIAL RESPONSIBILITIES:
Collects DSME/T program participant assessment data, using a recognized certification database/program in a collaborative and ongoing manner.
Develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants.
Collaboratively develops an individualized follow-up plan with each program participant. Evaluates effectiveness of educational services provided by measuring attainment of learning objectives.
Conducts a follow-up assessment upon completion of DSME/T program services, using outcome measures from the AADE7 Continuum of Outcomes Framework.
Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records.
Participates in the development of education curricula and materials.
Performs other duties as assigned or required.
QUALIFICATIONS:
Minimum
Bachelor’s Degree
Registered/Licensed Dietitian.
Certified Diabetes Care and Education Specialist (CDCES) or eligible and must obtain CDCES certification within nine months of hire
Experience in counseling patients with diabetes and experience in community education.
Act 34 Criminal Background Clearance Certificate
Act 33 Child Abuse Clearance Certificate
Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
Preferred
Master’s Degree.
1-3 years’ experience in Diabetes Education / Health Living Coaching.
Certified insulin pump trainer (CPT).
