Our programs build upon, and tie together, the Hospital’s inpatient/ED care coordination, Ambulatory Care Network, and community outreach programs. Now, we're seeking a Care Management Coordinator to help drive this work forward and help us improve Quality of Care for our patients.

RN Care Manager - (HIV) Comprehensive Health Program/Population Health - Columbia Ambulatory Care Network - Grant Funded

The Comprehensive Health Program (CHP) provides integrated HIV and Sexual Health care, behavioral health, and supportive services for people living with or at high-risk of HIV as part of a comprehensive team-based care model.  CHP works with city and state agencies as well as with local community-based organizations to deliver new medical interventions (immediate ART, PrEP/PEP, and comprehensive STI testing/screening) and care coordination services to link clients to essential service in support of their health and mental wellbeing. The RN Care Manager role collaborates with team members to provide care management and expedite essential care coordination support to high-risk patients to improve health outcomes and avoid unnecessary emergency and inpatient admissions. Services may be performed in person or telephonically. Assessments and interventions are recorded in the electronic health record. The RN Care Manager will also work closely with programmatic leadership and other RN Care Managers to drive Quality Improvement activities program-wide and hospital-wide. 

Monday – Friday 9 AM – 5:30 PM.

Preferred Criteria

  • Quality Improvement experience
  • Leadership experience in either inpatient care management, community care management, or outpatient/ambulatory care case management

Required Criteria

  • BSN
  • Current NYS RN Licensure
  • Minimum 5 years of healthcare experience
  • Experience with electronic medical records
  • Computer proficiency in Word and Excel
  • Excellent interpersonal skills
  • Ability to problem solve and prioritize
  • Flexibility and ability to work in fast-paced, changing environment

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

NYP will not reimburse for travel expenses.
