Pay Range

Rate: $26.60 - $32.54 /visit (placement on salary range as per Collective Agreement) + $5.00 Top Up Premium Per Visit
 

Estimated annual earning potential: $70,000–$120,000. Actual earnings may vary based on availability and service volumes   


 

Grounded in our commitment to Client and Family Centered Care the key foci on the RPN role is to provide and coordinate home health nursing services to diverse group of individuals, families and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, the program where they provide services, and the Standards of Practice in Community Health Nursing.

We are seeking Part-Time Registered Practical Nurses (RPN) to join our Hospital Partnership Program. Nurses hired for this role will work in the North York or Toronto Central region. These shifts are days (0900 - 1300) and evenings (1300 - 2100) with bi-weekly weekend rotation. A valid Ontario driver’s license (G/G2 License) and a reliable vehicle are required for this role.

The Hospital Partnership Program focuses on providing care to clients for 90–120 days following discharge from the hospital to home. The program is designed to ensure that care is delivered in the client’s home at a pace that is personalized and comfortable. Nurses conduct comprehensive head-to-toe assessments and deliver services such as wound care, IV therapy, chest tube management, drain care, and diabetes management, among others. Care is coordinated in collaboration with hospital partners, including ICL Leads at UHN and Home Care Specialists from VHA, to regularly review and align care plans with each client’s evolving needs.

Key Areas of Accountability

VHA's Registered Practical Nurses are required to do the following tasks, but not limited to:

  • Assess, monitor, and make care planning and care coordination.

  • Evaluate care plan interventions through on-going reassessment and evaluation results.

  • Ensure the quality of patient care by identifying and acting on factors which affects practice autonomy and delivery of quality care.

  • Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.

  • Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.

  • Inform the nursing supervisor of any client activities, relevant concerns, or changes in the client’s condition or care plan, and submit appropriate documentation to the case coordinator.

  • Maintain documentation standards according to the College of Nurses of Ontario.

  • Participate in a standby/on-call rotation to be available for answering calls and respond, if needed.

  • Participate in team meetings, as required during the stated availability.

Required Qualifications:

  • Registered and in good standing with the College of Nurses of Ontario (CNO).

  • Membership of your Professional Association preferred (RNAO/WeRPN).

  • RPN diploma/degree from a recognized university or college.

  • Valid First Aid or CPR Certification.

  • Acute Care and Medical-Surgical nursing experience is an asset.

  • Knowledge of community nursing is an asset.

  • Demonstrate effective communication (written and verbal) and documentation skills.

  • Demonstrate reliable attendance, punctuality, and availability for the role.

  • Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.

  • A valid Ontario driver’s license (G2 or G License) and a reliable vehicle for traveling are required.
