Day Shift - 7.5 Hours (United States of America)
•Outreach and engage individuals at risk of overdose.
•Connect clients to harm reduction services including but not limited to naloxone training and kits, fentanyl test strips, etc.
• Complete or document clinical assessments, treatment and care planning activities, andcontinuing care management per ASAM guidelines
• Use SI PPS systems to track outreach and engagement activity; conduct Health Related
• Social Needs (HRSN) assessments using SI PPS technology and make referrals totreatment and recovery services
• Ensure each participant signs a 42 CFR compliant universal consent, documented electronically using SI PPS systems, so information can be shared between participating agencies and SI PPS
• Track and report outreach, engagement, and outcomes and report to SI PPS on a quarterly basis
• Participate in biweekly case conferences to review outreach and engagement activities
Requirements:
- Highschool Diploma
-Certified Recovery Peer Advocate (CRPA) Required
Monday - Friday 9a-5p
Salary Range: $25.134/hr - $26.461/hr
