Hospice Home Care LPN
Location: Waterbury Region
Status: Full Time Home Care

Position Purpose:
Our hospice LPNs use cutting edge technology and clinical knowledge to provide exceptional, compassionate care to patients with life-limiting or terminal illnesses in their homes. Hospice nurses are responsible for patient symptom control and overall supervision of patient care and outcomes while reporting findings to the RN case manager. They provide support and comfort, focusing on helping patients manage their pain. Our nurses follow physician orders and act in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.
Position Details:
Schedule: Mon - Fri, 8 am - 5 pm, occasional backup shift every 2 – 3 months
Region: Waterbury Region
NOTE: This position is 100% home care providing services in patients’ private homes.
What you will do:
Plan and prioritize visits based on client needs and physician orders.
Maintain patient care plans with measurable goals in collaboration with RN case manager, physician, and client.
Provide therapy using approved techniques, exercises, and modalities.
Coordinate care and referrals with other disciplines as needed.
Educate clients and caregivers on therapy programs and home exercises.
Monitor progress and report changes promptly to the RN case manager and physician.
Ensure compliance with laws, regulations, and agency policies.
Complete documentation at point of care in a timely manner.
Minimum Qualifications:
Graduate of an approved nursing education program
Licensure as a Licensed Practical Nurse in the State of Connecticut
One (1) year experience as a professional acute care nurse
Hospice home care experience preferred but not required
Ability to work with minimal supervision and maintain professional relationships with clients and families
Valid driver’s license and reliable transportation required
