What You’ll Do

Deliver Compassionate Hospice Care

• Provide direct patient care in accordance with the state Nurse Practice Act, including end-of-life care
• Implement and support individualized plans of care under RN supervision
• Deliver effective pain and symptom management, evaluating patient response to treatment
• Educate patients and caregivers on disease progression, comfort measures, and care needs
• Maintain a safe, clean, and supportive care environment

Collaborate & Communicate

• Document care accurately and timely in the EMR
• Partner with physicians and interdisciplinary team members (RN, HHA, Social Work, Chaplain, etc.)
• Participate in care coordination and team discussions to ensure holistic patient support
• Build trusted, compassionate relationships with patients and families

• Uphold the highest standards of clinical practice and professionalism
• Participate in quality improvement and team initiatives
• Engage in ongoing professional development

What You Bring

• Active LPN/LVN license in the state of practice
• Current CPR certification
• 1–3+ years of nursing experience preferred (hospice, home health, or med-surg a plus)
• Strong communication skills and emotional intelligence
• Ability to provide compassionate, patient-centered end-of-life care
• Valid driver’s license, reliable transportation, and auto insurance
