About the role
The Pharmacy Medication Reconciliation Technician provides support to the patient care team by collecting an accurate patient medication history from various sources and updating the EPIC medication list. The position interacts with patients, family, and other Healthcare providers, as appropriate and under strict accordance with Franciscan Alliance policies and procedures.

DAYS / EVENINGS. SHIFTS RANGE FROM 730AM TO 10PM. EVERY 3RD WEEKEND.
WHAT YOU CAN EXPECT
Dispense medications following the organization's policies and procedures.
Interview patients and/or family members to obtain information about current medications prescribed and taking, adherence to the documented regimen, and medication changes.
Maintain drug and supply inventory in distribution areas.
Package and accurately label extemporaneous solid and liquid dosage medications, in compliance with state and federal regulations.
Participate in the department’s Quality Improvement Program, and other continuous and process improvement initiatives.
Utilize EPIC to access information and document completion of tasks.
QUALIFICATIONS
Required High School Diploma/GED
2 years Pharmacy Tech (hospital or retail) Preferred
Pharmacy Intern Permit (PHINTERN) - State Licensing Board
Pharmacy Tech, State Cert (PHT) - State Licensing Board Required
Pharmacy Technician Certification Board (PTCB) Certification - Pharmacy Technician Certification Board
