Job Title: Crisis Peer Outreach Worker
Department: Behavioral Health
Location: Clifton Springs Hospital: 2 Coulter Road Clifton Springs, NY 14432
Hours Per Week: 40 / week
Schedule: Full time
SUMMARY:
Position is responsible for providing support to individuals who are experiencing a mental health crisis.
RESPONSIBILITIES:
• Offer hope through connection and engagement on what is possible
• Support individuals in connecting or reconnecting to their community and relationships
• Connect to and support individuals in various stages of navigation from CPEP to community, based on identified needs and
requests for support
• Provide validation and support through person centered approaches and engagement
• Advocate when requested by the individual
• Assist in the development of comfort around self-advocacy
• Demonstrate personal self-help and wellness skills
• Share known resources and knowledge on system navigation
• Serve as a mentor when appropriate on wellness and recovery
• Assist in the exploration of personal goals that lead to life satisfaction
• Engage in shared decision making with individuals to support voice and choice in all treatment decisions
• Utilize evidence-based peer interventions such as Action Planning for Prevention and Recovery, Shared Decision Making,
Psychiatric Advanced Directives, Building Recovery Capital, and the Eight Dimensions of Wellness
• Conduct informational groups for CPEP staff about the benefits of working with Peer Bridgers
• Work collaboratively with the team to educate all involved on recovery, self-determination, shared decision making, dignity of risk,
and peer values
REQUIRED QUALIFICATIONS:

REQUIRED LICENSURE/CERTIFICATIONS:
One of the following certifications through the New York Peer Specialist Certification Board (NYPSCB) or New York Certification Board (NYCB) required at hire:
New York Peer Specialist Certification Board(NYPSCB).
• New York Certified Peer Specialist (NYCPS)
• New York Certified Peer Specialist, Provisional (NYCPS-P),
***If New York Certified Peer Specialist, Provisional (NYCPS-P), must obtain full certification within 6 months of hire date.
New York Certification Board (NYCB)
• New York Certified Recovery Peer Advocate (CRPA)
• Certified Recovery Peer Advocate-Family (CRPA-F)
• Certified Recovery Peer Advocate-Youth (CRPA-Y)
***If Certified Recovery Peer Advocate Provisional (CRPA-Provisional), must obtain full certification within 6 months of hire date.
PREFERRED QUALIFICATIONS:
Knowledge of and ability to navigate community resources
• Ability to model, facilitate, and convey the message that healing is possible and probable
• Proficiency in oral and written communication, reading comprehension, and computer literacy, including foundational knowledge of
Microsoft Office applications such as Teams, Word, PowerPoint, Excel, and Outlook.
• Demonstrate excellent interpersonal and communication skills with ability to work independently and as part of a team

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
