Job Summary:

The Licensed Practical Nurse (LPN) in Home Health or Hospice will provide nursing care for patients following established standards and practices in accordance with the South Carolina Nurse Practice Act. The LPN will apply knowledge of patient care tasks and techniques to provide nursing care.

Minimum Qualifications:

Education: Graduate of an accredited school of practical nursing.

Experience: At least 1 year of clinical practice experience as an LPN preferred.

Licensure/Certification: Currently licensed as an LPN in the state of South Carolina or hold a current compact/multi-state license as a Licensed Practical Nurse in a recognized NCSBN Compact State and is not a permanent resident of SC. Must have a current American Heart Association BLS for Healthcare Provider Card.

Knowledge/Skills: Knowledge of organizational policies and procedures to provide patient care. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in preparing and maintaining records. Skill in establishing and maintaining effective working relationships. Ability to communicate clearly. Knowledge of common safety hazards and precautions to establish a safe work environment. Demonstration of required competencies as assigned by a supervising Registered Nurse.

Constant interaction with internal and external customers to include (but not limited to) patients, families, physicians, employees and visitors.

Work Demands/Environment:

Frequent standing, walking, bending, crouching, stooping. Frequent lifting/moving patients. Intermittent lifting, moving or carrying, pushing or pulling up to 50 lbs. Frequent use of finger/hand dexterity. Constant reaching with hands/arms. Frequent talking or hearing. Corrected hearing and vision to normal range. Exposure to blood, body fluids or tissue. Possible exposure to infectious materials, communicable diseases and/or other conditions common to a healthcare environment. Intermittent exposure to risk of exposure to blood-borne pathogens. Moderate noise level in work area. Requires work under stressful conditions, deadlines, or irregular hours. May be required to take call and/or work weekends/holidays based on the needs of the department.
