Registered Nurse - Hospice

The Hospice Admissions RN is responsible for all hospice referrals in the hospital setting. Completing all processes to admit medically eligible patients to our hospice program in a variety of locations (hospital, home, nursing homes, ALF, etc.)  

- Graduated from an accredited nursing program
- Valid Florida license for Registered Nurse (RN)
- Valid driver’s license with a good driving record; Personal car in good working order
- Minimum 1 year experience in Medical Surgery or equivalent
- Hospice experience preferred  
- Providing information/education about hospice services, end of life care, advance directives, treatment options vs. comfort care, hospice eligibility criteria, levels of care, and financial responsibility to all referrals; Admit referrals who meet eligibility
- Performing patient assessment and documentation of patient eligibility and care needs
- Completing admission process
- Ensuring patient and patient care giver have all immediate care needs for services, equipment, medication and supplies
- Providing immediate needs for pain and symptom management
- Coordinating safe transfer from the hospital to alternate care locations
- Perform patient assessment and documentation of the ongoing General In Patients and assist with transfer if applicable
- Orienting new staff
