The Home Health Hospice Aide, under the supervision of a licensed clinician, participates in the delivery of patient care in the home or inpatient setting.

WHAT YOU CAN EXPECT
Monday to Friday- 8am to 5pm
Assists patients with care and activities of daily living as directed in the established Plan of Care
Communicates any change in condition or lack of patient progress.
Completes necessary documentation of care provided. Records intake, output, and vital signs as directed and communicates care as appropriate.
Provides assistance with personal hygiene and elimination needs according to the established plan of care.
Schedules visits with the patient or caregiver.
Assists patients with ambulation and transfer using proper body mechanics.
QUALIFICATIONS
High School Diploma/GED - Required
1 Year Long Term Care Setting Experience- Preferred
1 Year Home Healthcare Experience - Preferred
1 Year Hospice Experience - Preferred
Certified Nursing Assistant (CNA) OR Home Health Aide (HHA)- State Licensing Board - Required
Cardiopulmonary Resuscitation (CPR)- American Heart Association - Required
Auto Insurance - Required
Driver's License - Required
