Overview

Otterbein has an employment opportunity for an Assistant Director of Nursing - Transitional Care Unit. The ADON is a critical member of the health care leadership team responsible for many aspects of the clinical leadership including care planning and meeting with families regarding the care of their loved one.

Due to the COVID-19 pandemic, we supply PPE (Personal Protective Equipment) to keep you and our residents safe. Additionally, we have implemented stronger infection control and testing procedures in all Otterbein locations.

Responsibilities

  • Effectively complete the Care Plans
  • Track Physician and Ancillary Visits and Orders
  • Discuss resident observations/concerns with appropriate coordinator
  • Complete wound rounds and work with the physicians to coordinate wound care
  • Provides coaching and direction to staff as needed
  • Complete discharge planning for residents including home services upon discharge
  • Complete the certifications and recertification’s for residents in the skilled nursing unit
  • Participate in the weekly Medicare/Therapy/Resident meetings
  • Act as the therapy liaison
  • Complete the reassessments for residents and hospital visits when appropriate
  • Facilitate the restorative program
  • Participate in the morning stand up meetings
  • Assist in the management of the survey process
  • Assist with doctor rounds
  • Serve as the “on-call” nurse in charge on a rotating basis
  • Perform any other duties as assigned.

Qualifications

  • Education: Graduate of approved Registered Nursing program
  • Licensure/Certification: Valid Ohio Registered Nurse (RN) license
  • Experience: Minimum of three (3) years’ experience as a nurse in long term care. Experience with MDS, Restorative Nursing and/or as a nursing supervisor is highly desirable.
  • Computer Skills: Sufficient level computer skills to perform electronic medical record/submission duties

  • Demonstrate knowledge of clinical standards of practice, regulations and reimbursement governing long-term care and Medicare/Medicaid participation.
  • Recognize the autonomy and dignity of all residents.
  • Communicate a sense of caring, concern, and dignity for residents.
  • Understand how to place decision-making in the hands of the residents whenever possible and appropriate.
  • Make prompt and accurate judgments with regard to resident care and emergencies.
