Basic Function

The Patient Care Coordinator provides frontline administrative and clinical support to HIV primary care services, with a specialized focus on coordinating long-acting injectable HIV therapies. This role ensures seamless scheduling, medication tracking, patient engagement, and operational support to promote continuity of care and optimize viral suppression outcomes. The ideal candidate is highly organized, patient-centered, and experienced in clinical coordination within a fast-paced healthcare setting.

Nature and Scope

External contacts include a variety of third party payers, medical organizations, patients and other healthcare personnel

Principal Accountabilities

  • Maintain scheduling and tracking systems for long-acting injectable HIV medications.

  • Coordinate with clinical staff and specialty pharmacies to ensure timely medication procurement and dosing.

  • Monitor injection schedules and proactively address potential delays or barriers to care.

  • Assist with patient reminders, appointment confirmations, and follow-up outreach to reduce missed visits.

  • Perform patient check-in and check-out procedures.

  • Verify insurance eligibility and obtain necessary authorizations.

  • Coordinate referrals and follow up to ensure continuity of care.

  • Document patient interactions and care coordination activities in the Electronic Medical Record (EMR).

  • Support reporting initiatives related to patient engagement and viral suppression outcomes.

  • Collaborate with providers, nurses, case managers, and pharmacy partners.

  • Maintain accurate and confidential patient records.

  • Assist with daily clinic workflows to ensure efficient operations.

  • Support quality improvement initiatives and performance reporting.

Minimum Requirements

Current certification as a Medical Assistant (CMA or equivalent).

High school diploma or equivalent (Associate degree preferred).

Minimum 1–2 years of experience in a clinical or healthcare setting (HIV or infectious disease experience preferred). Experience with EMR systems.

Strong organizational, communication, and time-management skills.

Ability to manage multiple tasks in a fast-paced environment.
