Day Shift - 7.5 Hours (United States of America)

The Licensed Social Worker provides clinical services to pediatric and adult patients and their families in the outpatient Family Health Centers, which operate under a patient-centered medical home (PCMH) model. This role involves conducting assessments, delivering individual, family, and group therapy, coordinating care with interdisciplinary team members, and ensuring compliance with applicable laws and regulations.

 

Essential Duties and Responsibilities:

  • Provide individual, family, and group therapy for pediatric and adult patients in an outpatient setting.
  • Conduct psychosocial assessments and develop treatment plans tailored to patient needs.
  • Collaborate with medical providers, behavioral health specialists, and other interdisciplinary team members to ensure integrated patient care.
  • Facilitate referrals to community resources, including mental health services, housing assistance, financial aid, and social support programs.
  • Maintain timely and accurate documentation in the electronic medical record (EMR) in compliance with NYS law, NYC contractual obligations, and agency policies.
  • Ensure compliance with NYS Mental Hygiene laws and other regulations related to patient care, clinical records, and mandated reporting of child abuse/neglect.
  • Participate in team meetings, in-service training programs, and community outreach efforts as required.
  • Meet monthly productivity and documentation requirements in accordance with organizational policies.

Qualifications:

  • LCSW required.
  • Experience providing clinical services to children, families, and adults in an outpatient or inpatient setting.
  • Proficiency in using electronic medical records (EMR).
  • Spanish-speaking preferred.
  • Ability to work daytime, evening, and Saturday daytime hours as required.

Physical and Environmental Demands:

  • Work is primarily performed in an outpatient clinic setting at multiple clinic locations
  • May require prolonged periods of sitting, standing, or walking.
  • Requires ability to use a computer for documentation and communication.

 

May be required to work any shift between 8:30AM - 8PM, Monday-Friday
Salary Range: $39.683/hr - $48.430
