JOB DESCRIPTION:
The Outpatient Social Work Case Manager (SWCM) is a dynamic, split position that supports multiple service lines, including dialysis, nephrology, and transplant. This role will provide outpatient social work case management services to transplant patients participating in the Dartmouth Hitchcock transplant program. As part of a contracted partnership, our team offers local, satellite support to these patients, helping ensure continuity of care close to home.
This role is within the interdisciplinary care team, which delivers care to patients in a longitudinal rather than episodic fashion and focuses on prevention, health promotion, care coordination, and chronic disease management. The SWCM is responsible for assessment and intervention with patients and families across the healthcare continuum when psychosocial and/or high-risk factors impact or interfere with treatment plans or health outcomes. The SWCM utilizes advocacy skills, provides therapeutic and supportive interventions, offers resources and referral information, and delivers financial guidance and assistance as needed.
The position offers a unique and engaging mix of responsibilities, with day-to-day work varying based on patient needs across these specialty areas. While the role is primarily based locally and does not typically require significant travel, occasional travel to Dartmouth Hitchcock may be required for training or on a limited, as-needed basis.
EDUCATION:
MSW from an accredited school of social work required. Licensure through the State of Vermont is preferred.
EXPERIENCE:
Greater than 1 year experience working in a human service organization, health care setting, or managed care system is required.
