About the role
The Screening Coordinator provides screening services at Franciscan Health (Heart Scans, Lung Scans, Vascular Screenings, Cholesterol Screenings, Glucose Screening, Blood Pressure, and Body Fat). The Screening Coordinator provides education and counseling to patients in the Screening Center on health maintenance, disease prevention, early detection, and guides patients to follow-up care within the Franciscan System. The coordinator also works as a liaison and builds effective working relationships with patients, healthcare providers, and staff to reduce barriers to patient care, meet patient needs, and improve patient health outcomes.

WHAT YOU CAN EXPECT
Schedule: Tuesday - Thursday, 7:30am - 4:00pm
Educate patients on screening results, preventative health maintenance, smoking cessation and NRT. Review all radiology reports for incidental findings and makes appropriate referrals.
Document accurate documentation of each patient encounter. Ensure all patient records are accurate, up to date and meet all regulatory and confidentiality guidelines.
Perform patient outreach regarding required follow-up testing and screening compliance.
Schedule and coordinate patient exams and follow up with healthcare professionals and patients regarding the results.
Answer questions as appropriate and serve as liaison to the physician for additional questions and concerns. Communicate all findings to the patient’s primary care physician.
Monitor screening guidelines and compliance according to the Franciscan system and national guidelines. Inspect the area for compliance with policies, procedures, regulations, and standards and takes corrective action when violations occur. Ensure accurate data submission into appropriate database systems.
Suggest policies or practices that should be implemented to support increased quality of care; utilize evidenced-based protocols in developing policy and procedures.
Collaborate with FPN and Hospital administration to ensure physician buy-in and seamless patient continuum of care.
Collaborate with vascular/cardiac/thoracic surgeons, cardiologists, oncologists, pulmonologists, radiologists, hospital administration and physician offices to identify growth opportunities for the screening program.
May be required to maintain screening supplies inventory by checking stock and outdates to determine inventory level, anticipating needed supplies and placing orders for required inventory.
QUALIFICATIONS
Bachelor's Degree in Health Promotion, Exercise Science, Kinesiology or Public Health required
1 year of Community Health experience required
1 year of Clinical Education required
1 year of Biometric Screening experience preferred
