Napa Post Acute is Hiring an LVN MDS Coordinator!

Pay: $38–$45/hour
Schedule: Full-Time

Napa Post Acute is looking for a knowledgeable and detail-oriented LVN MDS Coordinator to join our dedicated team. If you’re passionate about quality care, thrive in a collaborative environment, and have a strong understanding of the MDS process, we’d love to connect with you.

What You’ll Do:

Coordinate and complete MDS assessments in accordance with federal and state regulations
Ensure accurate and timely submissions
Collaborate with interdisciplinary team members to support care planning
Monitor documentation to ensure compliance and optimize reimbursement
Support clinical staff with MDS-related processes and education

What We’re Looking For:

Current California LVN license
MDS experience preferred (or strong clinical background with willingness to learn)
Detail-oriented with strong organizational skills
Ability to work independently while being a team player
Knowledge of PDPM and regulatory requirements is a plus
