Napa Post Acute is Hiring an LVN MDS Coordinator!
Pay: $38–$45/hour
Schedule: Full-Time
Napa Post Acute is looking for a knowledgeable and detail-oriented LVN MDS Coordinator to join our dedicated team. If you’re passionate about quality care, thrive in a collaborative environment, and have a strong understanding of the MDS process, we’d love to connect with you.
What You’ll Do:
- Coordinate and complete MDS assessments in accordance with federal and state regulations
- Ensure accurate and timely submissions
- Collaborate with interdisciplinary team members to support care planning
- Monitor documentation to ensure compliance and optimize reimbursement
- Support clinical staff with MDS-related processes and education
What We’re Looking For:
- Current California LVN license
- MDS experience preferred (or strong clinical background with willingness to learn)
- Detail-oriented with strong organizational skills
- Ability to work independently while being a team player
- Knowledge of PDPM and regulatory requirements is a plus
