Job Description:

To plan, implement, coordinate, and manage care services to ensure safe and effective patient care.

Scope

Responsible for care management, and other related programs within an assigned Clinical Program, functioning with collaborative implementation and evaluation of Clinical Integration Programs, Care Process Models, Care Management, Utilization Management and Patient Teaching. Responsible for quality functions and providing facilitation of positive outcomes.

Job Essentials

Provides for program development and implementation by researching evidence based clinical practice processes, develops strategies and action plans to ensure program goals reflect best practice.
Accountable for care coordination including assessment, individual plan of care, education, patient involvement and choice, resource coordination, ongoing monitoring and care conferencing according to hospital policy.
Assists health care providers in collaborative interdisciplinary teaching for diagnostic specific patients, providing materials and literature where appropriate.
Ensures effective planning and arranging for needed services upon discharge. Evaluates the effectiveness of discharge process by monitoring 30-day readmissions. Completes necessary teaching fir identified clinical program.
Evaluates patients for meeting appropriate admission criteria and appropriate level of care.
Identifies and provides care management for patients with complex needs, prolonged length of stay and/or frequent utilization of acute care services.
Monitors and evaluates the effectiveness of care management activities in reaching desired patient outcomes on an ongoing basis.


Minimum Qualifications

RNs hired or promoted into this role need to have or obtain their BSN within four years of hire or promotion.

Current RN Licensure in the State of Utah.

Basic Life Support (BLS) Certification

Three years clinical hospital experience.

Exceptional oral and written communication skills.
- and -
Strong ability for conceptual thinking, problem resolution and planning skills.

Preferred Qualifications

Bachelors degree in Nursing (BSN) from an accredited institution (education will be verified).

Masters degree.

Clinical Project Management experience.

Data Management experience.

Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess patient needs.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
- and -
Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, etc.
