Join our multidisciplinary care team as a Social Worker II, where you’ll play a vital role in supporting patients and families through complex medical, emotional, and social challenges. This role offers the opportunity to make a meaningful impact by guiding patients across the continuum of care, advocating for their needs, and ensuring safe, effective discharge planning.
Essential Functions
- Identifies patients who would benefit from Case Management interventions through initial screening and assessment of discharge needs
- Conducts comprehensive interviews with patients and families to gather relevant psychosocial, emotional, and environmental information
- Completes psychosocial assessments in accordance with departmental, regulatory, and professional standards
- Develops, implements, and evaluates individualized psychosocial treatment plans utilizing appropriate social work modalities, including crisis intervention, individual and family therapy, and grief and bereavement counseling
- Initiates and coordinates appropriate internal and external referrals based on patient-specific needs
- Advocates for patients to address unmet needs and remove barriers to care
- Collaborates with the multidisciplinary team to develop and implement effective discharge plans that promote continuity of care
- Coordinates and facilitates patient and family care conferences as needed
- Identifies barriers to efficient and effective patient care and seeks strategies to eliminate those barriers
- Maintains current knowledge of federal, state, and local regulations impacting social work practice
- Demonstrates awareness of payer and reimbursement practices and guides patients and families through post-discharge care options
- Documents all assessments, interventions, and care coordination activities in the Electronic Medical Record in accordance with departmental standards
Education, Experience, Licensure & Certification
- Master’s degree in Social Work from a school accredited by the Council on Social Work Education (CSWE)
- Clinical licensure (or provisional status) preferred
- Certification as an Accredited Case Manager (ACM-SW) preferred
- Social work experience in a healthcare setting strongly preferred

- Ability to establish positive working relationships and contribute to a collaborative team environment
- Proven ability to manage multiple priorities in a fast-paced setting with frequent interruptions
