Day Shift - 7.5 Hours (United States of America)

Job Summary:

The Social Worker will provide clinical and case management care to patients with intellectual and or developmental disabilities across the life span in an outpatient setting. Documentation must be in a timely and quality manner. Conduct individual, group, and family therapy. Assess, identify, and provide services for patients/families; address psychosocial concerns as well as post hospital care, including crisis intervention, advocacy and community referrals.

Education:

Master's degree required.

Licenses/Certifications:

NYS Registration/Certification as a Social Worker (LMSW/LCSW) is required.  Membership in the NASW Academy is a plus.

Experiences:

2 yrs post graduate work preferred.  Prior experience in a hospital or community health agency is preferred.  Experience with assigned patient population is required.  Additional language fluency and computer literacy preferred.  Additional language fluency and computer literacy preferred. Experience treating concurring substance abuse and mental illness preferred.

Knowledge, Skills, & Abilities:

Knowledge of medical/health related setting and/or specialty area experience preferred.

Monday, Wednesday & Friday 9a-5p; Tuesday & Thursday 11a-7p

Salary Range: $39.683/hr - $48.430/hr
